Hospital to Home Service Launch 1 January 2008

The Hospital to Home Service was officially launched by Hon W E Teare MHK, Minister for Health and Social Security on 30th January, 2008.

The Hospital to Home Service is a pilot scheme based in Southlands Resource Centre, operated by the Social Services Home Care Team, to support people over the age of 65 to successfully make the transition from a hospital stay back to their own home.

Often older people who are discharged from hospital find themselves in residential care for extended periods, sometimes permanently, for lack of a little support to help them regain the confidence and skills to live at home again.

The Hospital to Home Service provides this support. An Occupational Therapist or Physiotherapist will draw up a personal plan, along with the client, of the skills they will require to remain in their own home. Clients then transfer to the Bradda Unit for a brief period before returning to their own home with continued support from the team at the Hospital to Home Service.

Referral is via the Hospital Social Work Team at Noble’s, supported by either an Occupational Therapist or Physiotherapist. The service is free to clients who meet the criteria for admission. Since September 2007 eight people have already used the service and successfully returned to their own homes. The Hospital to Home Service is helping the Department of Health and Social Security meet its goal of providing quality services to those in need and creating choice and diversity. It enables clients to maintain their independence and leave hospital sooner than might previously have been the case and return to their own homes.

This service is one of a number of services now provided for the community in Southlands Resource Centre, the others being; residential care, residential care in a safe environment for clients with memory problems, EMI care, short term care and day services for the older person, including EMI.
