New FSC Questionnaires 29 June 2004

The Financial Supervision Commission has reviewed and updated the Personal Questionnaire and Bankers Questionnaire currently used to approve the appointment of individuals to positions as Manager, Director, Controller or Key staff of a licence holder or licence applicant.

The changes are largely presentational and should make it easier for individuals and organisations to complete the questionnaires and to provide the Commission with the information it requires to assess applicants. In addition, a number of questions and statements have been refined in order to clarify the nature of the information required.

The Commission has also introduced a new declaration to be used in instances where an individual requires approval for a new role and it has been less than 5 years since they previously completed a Personal Questionnaire. This form will replace the 'no material changes' letter.

The new Questionnaires and Declaration will be available for use via the Commission's website www.fsc.gov.im from 01 July 2004 when the current versions will be removed. They can be accessed via the Library section from the front page and then by clicking on Forms on the left hand side within that section.

The Commission will accept the "old" versions of the forms until 01 September 2004 after which time the new forms must be used.
